(Purpose:to inspire students'interest in doing the survey by providing students with a real situation and giving clear instructions.) Step 2 Design a multiple-choice questionnaire Activity 1 Narrow down the topic of your surveyStudents list specific aspects of physical and mental health,such as diet.Then each group chooses one issue that they are most interested in. (Purpose:to help studentsfind afocusfor their survey.) Activity 2 Define survey goals Ask students what specifically they want to find out.Then the teacher gives an example to help students understand it better. eg.Diet whether your classmates'eating habits are healthy or not; ifnot,what are the causes for these unhealthy habits. (Purpose:to guide students to set a clear and definite goalfor their survey.) Activity 3 Design multiple-choice questions The teacher gives directions in designing questions: use multiple-choice questions ● directed toward survey goals Take“diet”as an example. Question 1:What are the eating habits that affect their physical health eg.skipping meals,eatingjunk foods and drinks,eating too much snacks,eating little fruits and vegetables... Based on this,students can ask questions like: EXAMPLE How often do you skip meals A.never B.once a day C.no more than three times a week D.others: Question 2:What are the possible causes behind these habits eg.lack oftime,to lose weight,no desire forfood... Step3 Distribute and collect questionnaires Activity 4 Groups distribute either paper questionnaires or e-questionnaires among classmates.
