Deo Kato, an athlete, has run thousands of miles to help make the changes he wants to see in the world.
Kato was born in Uganda, and moved to England at age 10. He’s used to running, and normally works as a running coach.
On December 22, Kato became the first person to run from Cape Town, South Africa to London, England. Kato wanted to call attention to the struggles of immigrants (移民), and the racism they often face.
In 2020, Kato began a 381-day program of running at least 6.2 miles (10 km) daily. That effort was meant to call attention to racism. He chose 381 days because that’s how long African Americans were stopped by the bus system in Montgomery, Alabama in the 1950s, fighting to be treated fairly. Kato hoped the run from Cape Town to London would also take 381 days. Instead, the roughly 8,000-mile (12,875-km) trip wound up taking 518 days. He set off on July 24, 2023, starting at a monument honoring Nelson Mandela. For the next year and five months, Kato ran north through Africa and Europe.
He ran through dangerous places where wars were being fought. At times he got sick. He was even put in prison in South Sudan. He also faced racism especially in Europe. But Kato also had many good experiences. In some places children ran along with him for miles. Lots of supporters showed up to greet him when he finally arrived in London.
Recently, Kato was asked if he would encourage anyone else to try it. “Definitely not,” he replied. But that doesn’t mean he’ll give up running. In September, he’s planning on running a 153-mile (246-km) race in Greece.

For years, there has been a push to recognise obesity (过度肥胖) as a disease itself rather than just as a risk factor for other things, such as diabetes, heart disease, strokes and some cancers. While obesity can lead to poor health, many obese individuals remain healthy. This suggests obesity alone should not be considered an illness.
Until two years ago, treatments for obesity were limited to bariatric surgery (减肥手术) or old- fashioned approach of eating less and exercising more. However, the arrival of GLP-1 weight-loss drugs, such as semaglutide (Wegovy), changed that. These drugs raised a question about who should be treated for obesity.
A group of 56 doctors, known as the Lancet Commission, developed a better way of diagnosing (诊断) obesity, distinguishing when it should be treated as a disease. Traditionally, obesity is measured by body mass index (BMI), calculated by dividing a person’s weight by the square of their height. Obesity is defined as a BMI over 30. However, BMI does not take account of fat distribution or muscle mass, and some individuals with high BMIs show no signs of illness.
To diagnose their newly defined disease, which they call “clinical obesity”, the commissioners require two things: a third measure of body size (waist circumference, waist-to-hip ratio, or waist-to-height ratio) and objective signs of reduced organ function or difficulty in conducting daily activities. The 18 signs the commissioners have lit on include breathlessness, heart failure, joint pain, and dysfunction in organs like the liver, heart, kidneys, and reproductive systems. Those without these signs are classified as having “preclinical obesity”, indicating an increased risk of developing clinical obesity but not requiring immediate drug treatment.
Francesco Rubino, a commissioner and a professor at King’s College, London, believes describing obesity as a disease is a significant shift. The next task is to determine how many of the 1 billion people classified as obese under the old definition qualify as clinically obese under the new one, estimated to be 20%—40%. The Commission’s approach has gained support from 76 leading health organizations worldwide. However, how quickly it will be adopted in medical practice is another matter.

Questions over the extent to which animals can experience mental illnesses have existed for centuries. While pet owners and researchers have been wondering about what animals might be thinking, modern science and brain imaging are providing clearer insights. Experts now believe that many mammals, and even some birds, can feel anxiety in ways similar to humans, though those similarities likely only go so far.
Animal doctors and animal behavior researchers say animals can experience mental illnesses, but not in exactly the same way humans do. For example, while dogs and cats may show signs of anxiety, they can’t express their feelings like humans can. This means we need to rely on their behavior to identify problems. Animal doctors diagnose anxiety or uncontrollable disorders based on visible signs, but since they can’t read animals’ minds, it’s unclear how many truly experience mental states similar to humans.
University of Pennsylvania associate professor and animal doctor Carlo Siracusa believes animals “absolutely” can experience mental illnesses. He often works with pets showing aggression or destructive behavior caused by fear or suffering, similar to angst in humans. Both human and animal brains process emotions in the same area, despite differences in size and complexity. For instance, dogs that face neglect or major changes in their environment may show sudden aggression, and like humans, pets can develop uncontrollable behaviors.
But some mental illnesses, like schizophrenia (精神分裂症), seem unique to humans due to the brain’s complexity. Studies suggest such conditions may result from specific DNA changes unique to humans. Animals don’t appear to feel social insecurity or hopelessness over abstract ideas, so they likely don’t experience certain types of depression. Siracusa explains this may be because animals lack advanced brain functions for long-term planning. Whether animals have self-awareness remains a debated and unresolved question.
