In my opinion, the summary includes all the necessary parts of survey results: the questions, the purpose, information about the people in the survey, results, personal opinion on the results and some comments on surprising results.

Our survey is about students’ after-school activities. We do the survey becasue we want to plan a New Year Party. There are thirty students in our class. They are between 11 and 12 years old. Fourteen are girls and sixteen are boys. Here are the results of our survey:
