概述
请假条应该开门见山，但是得有礼貌地提出请假。应该简单明了地陈述请假的原因事由，并尽可能表示对此带来工作或学习上的不便的歉意。在假条的最后写上你希望获得准假的天数。

黄金模板：
注意：
1. 人称：要用第一人称。
2. 时态：通常用将来时。如果是病假条，原因往往是过去发生的事，如什么时间得感冒、受伤或发生事故等，一般用一般过去时陈述请假的原因。
3.右上角写请假日期，如果题目没要求，则不写日期。请假日期按从小到大写——几点钟，月，日。如：11:00a.m. Jan.13
