All the doctors insisted that he was badly wounded and (insisted) that he (should) be operated on at once.

(1) insist意为“坚决要求，坚持主张(做某事)”时，其后所接的宾语从句常用虚拟语气，即谓语动词用“(should＋)动词原形”的形式。should可以省略。
(2)insist意为“坚称，坚决认为”时，其引导的宾语从句不用虚拟语气。

(1)as excited ...as the children是“as＋adj.＋as...”的原级比较结构；
(2)whom在此引导非限制性定语从句；
(3)“as if ...”是引导方式状语从句，此处的从句用了虚拟语气。
在as if引导的从句中，当说话人认为句子所述的是不真实的或极少有可能发生或存在的情况时，常用虚拟语气。
as if引导的从句的虚拟语气用法：
①从句所述内容与现在事实相反，谓语动词用一般过去时(be动词的过去式用were)。
②从句所述内容与过去事实相反，谓语动词用“had＋过去分词”。
③从句所述内容与将来事实可能相反，谓语动词用“would/could/might＋动词原形”。

(1)see sb. doing sth.指“看见某人正在做某事”，强调动作正在进行。其被动语态为sb. be seen doing sth.。
(2)see sb. do sth.指“看见某人做了某事”，强调动作全过程。其被动语态为sb. be seen to do sth.。
(3)see sb./sth. done指“看到某人/某物被……”。和前面两者的区别在于：此处的宾语和do之间是逻辑上的被动关系，而前面两者的宾语与do之间是逻辑上的主动关系。
